Re: Albert Thomas Stafford MANLEY deceased Regulation 28 Report to Prevent Future Deaths

Thank you for your letter dated the 30th May 2022 in relation to the above. I note your request that consideration be given to the recommendations provided in your report, which are based upon the observations recorded by Wiltshire Police. As you have identified in your report, subsequent to the collision that took place on the 30th May 2021 a number of alterations to the road signs and road markings have taken place. Following your suggestion that consideration be given to the introduction of further road markings and review of the current signing arrangements, I can advise the site has been visited by experienced road safety professionals with these comments in mind. Whilst the observations regarding the traffic signs are appreciated, the review has concluded that the existing sign arrangements are representative of the road layout and offer motorists the correct information of the hazards they should expect when negotiating this section of road. Therefore, no further amendments to the road signs are proposed. With regards to your other observation that the existing signing could be supplemented using road markings depicting the characters ‘SLOW’, this is something that is considered to be appropriate and would be beneficial in assisting motorist to negotiate this section of road. I confirm that this work will be added to the Council’s road marking programme of works for the coming months.

I note the recommendations within the Police report regarding the removal of the hedgerow and your observations on the matter. The hedgerow is outside of the highway boundary and is the responsibility of the landowner, and at present it is not proposed to suggest the removal of the hedgerow for the reasons you mention, but we will continue to monitor the situation and enter discussion with the landowner if deemed necessary in the future. Investigation and Inquest
On the 7 June 2021 I accepted the transfer of an investigation into the death of Albert Thomas Stafford Manley who was known to his family as "Jim". I went on to open Jim's Inquest on the 14 June 2021 and concluded both his and his late wife's Inquest on the basis that their deaths arose out of the same incident on the 19 May 2022. I found that Jim's medical cause of death was as follows:­ 1 a) Multiple Traumatic Injuries. As regards how, when and where Jim came by his death I recorded as follows:­ Albert, known to his family as "Jim", died at Southampton General Hospital, Tremona Road, Southampton, Hampshire on Monday 31 May 2021 from multiple traumatic injuries. Jim's injuries were sustained when his Ford Mondeo collided on the opposite/northbound side of the highway with the front offside of an oncoming Land Rover Discovery on the morning of Sunday 30 May 2021 whilst both vehicles were travelling in opposite directions on the 83081 near To/lard Royal in Wiltshire at a bend and junction with a minor road leading to To/lard Green (OS Ref ST917 183). It is unclear as to why Jim's car crossed the centre of the highway and collided with the front of Land Rover Discovery as to whether he intended to exit the junction and head towards To/lard Green or misinterpreted the layout of the road at the junction.
