I am writing to you on behalf of Salford City Council in response to the Regulation 28 Report to prevent future deaths at the junction of A57 Cadishead Way and B5311 Fairhills Road following the fatal road traffic collision involving Mr. Draper and Mr. Wojdyl on the 23rd June
2021.

Within the report you have made a request for the Council to conduct a review of four issues relating to the layout, speed limit, restricted view and provision of traffic signals at the junction of Fairhills Road with Cadishead Way.

I would like to inform you that Salford City Council’s Collision Investigation Team have carried out a detailed investigation into the junction following the collision. This involved a site visit on the 30th of June 2021 with Greater Manchester Police’s Specialist Operations Roads Policing Unit to understand the circumstances of the collision and to review the operation of the junction. A desktop exercise was also undertaken to review the collision history and traffic survey data in the immediate vicinity of the junction.

A technical note has been appended to this letter containing the findings from the Council’s investigation covering the four issues you have identified along with the actions the Council have already undertaken, as well as those actions still to be implemented.

I would be grateful for your acknowledgement upon receipt of this letter and appended documents. I would also be open to receiving any further feedback you have in response to the Council’s proposals to prevent a future death occurring at this junction.

Investigation and Inquest
On the 12th of July 2021 I commenced an investigation into the deaths of Michael John Draper, 62 years, born the 23rd of May 1959 and Rafal Wojdyl, 43 years, born the 23rd of March 1978. The investigation concluded at the end of the inquest on the 9th of February 2022. The medical cause of death of Michael John Draper was: 1 a) Multiple Injuries The medical cause of death of Rafal Wojdyl was: 1 a) Severe Head and Chest Traumatic Injuries The conclusion of the investigation in relation to both deceased was Road Traffic Collision.

1) Michael John Draper and Rafal Wojdyl died in a road traffic collision which occurred on the 23rd of June 2021 on Cadishead
