_ [BRIEF SUMMARY OF MATTERS OF CONCERN] The location of this collision was the junction of Station Road with the A358 at Bishops Lydeard. heard evidence at the inquest that a roundabout had been proposed at this junction for a number of years and that by the time the inquest commenced works were about to start. My concern is that there was a sign erected at this junction indicating that there had been a number of accidents at the junction previously but was concerned as to whether the erection of such a sign would also necessarily involve a review of maximum speed for vehicles approaching such a junction. Investigation and Inquest
On 29/04/2016 commenced an investigation into the death of Sally Frances Eveleigh, 70 The investigation concluded at the end of the inquest on 21 September 2016. The conclusion of the inquest was to record a death by Road Traffic Collision At approximately 06.45 on the 27th April 2016 at the junction of Station Road with the A358 Ms Eveleigh pulled out on to the major road into the path of a van, the driver of which was unable to avoid a collision.
Circumstances of the Death
Mrs Eveleigh was involved in a fatal Road Traffic Collision_ Mrs Eveleigh was the sole occupant of her vehicle when she pulled out onto the main road in front of a van: Paramedics attended and Mrs Eveleigh was still trapped in the driver's seat of the vehicle. Death was confirmed at the scene
Action Should Be Taken
In my opinion action should be taken to prevent future deaths and believe the Highways Department Taunton Deane District Council have the power to take such action.
