Recommendation MAI-155: Obtain comprehensive accounts from commanders
From Manchester Arena Inquiry: Volume 2: Emergency Response, published 3 November 2022.
Recommendation
The Home Office, the College of Policing, the National Ambulance Resilience Unit and the Fire Service College should take steps to ensure that all emergency services understand the importance of promptly obtaining comprehensive accounts from commanders as part of the debrief process following a Major Incident.
In brief (written by this site)
The Inquiry asked emergency services bodies to ensure debriefs promptly gather full commander accounts. In February 2026, the Cabinet Office reported that the three emergency services had agreed version 3 of the Joint Operating Principles (JOPs) (Cabinet Office, Feb 2026). It stated that this document covers debriefing and lessons identified, captures several inquiry recommendations, and is reviewed annually, though obtaining accounts from commanders was not mentioned (Cabinet Office, Feb 2026). Earlier, in November 2025, the UK Government recorded that the recommendation had been accepted in full (Cabinet Office response, Nov 2025).
