The registered persons had failed to tell us about serious injuries occurring within the home.

The registered persons failed to ensure that risks to people's safety and welfare were properly assessed and action taken to mitigate risks. This included risks associated with people's health, in the environment, from lack of measures to control infection and in the way that medicines were managed.

There was a lack of effective and suitable systems for assessing and monitoring the quality and safety of the service and for identifying and managing risks.
