The Council was at fault for not carrying out a mental capacity assessment for Ms C sooner than it did, and for the missed care visits to Ms C. Because of the fault, Ms C suffered a lack of care, and it caused stress to her son, Mr D. The Council will apologise to Ms C and Mr D, make a symbolic payment, and issue staff briefings.
