Independent investigation into maternity and neonatal services across 12 NHS trusts in England, chaired by Baroness Valerie Amos. Interim report published 26 February 2026 examining capacity pressures, culture and leadership, racism and discrimination, poor accountability, estate quality, and workforce challenges. No recommendations made in this interim report; evidence gathering ongoing. The investigation engaged over 400 family members and received 8,000+ public submissions.

Independent Review of Gender Identity Services for Children and Young People: Final Report

Independent review commissioned by NHS England and NHS Improvement in 2020, led by Dr Hilary Cass (retired consultant paediatrician, former President of the RCPCH). Published April 2024. Examined gender identity services for children and young people provided by GIDS at the Tavistock and Portman NHS Foundation Trust. Found services had expanded rapidly without an evidence base; poor clinical governance; inadequate follow-up of patients; and an unclear rationale for puberty blocker prescribing. Made 32 recommendations. NHS England accepted all 32. GIDS closed March 2024 and was replaced by two new regional services.

Independent review commissioned by the Secretary of State for Health and Social Care in 2018 to examine how the healthcare system responded to patients harmed by three medical interventions: hormone pregnancy tests (Primodos), sodium valproate (epilepsy drug causing birth defects), and pelvic mesh implants. Led by Baroness Julia Cumberlege. Published July 2020. Found the healthcare system to be "disjointed, siloed, unresponsive and defensive". Made 9 strategic recommendations and 50 Actions for Improvement. Government rejected recommendations 3 and 4 (redress and compensation), accepted or accepted in principle the remainder.

Commissioned by the Secretary of State for Health in October 2012 to identify themes and lessons from 44 NHS investigations into allegations of abuse by Jimmy Savile on NHS premises. Published February 2015 with 14 recommendations covering celebrity/VIP access, volunteer management, safeguarding, DBS checking, complaints, fundraising governance. Government response published November 2015 accepted 13 of 14 recommendations in principle.
