It is vital that the Department develop a culture of learning from its mistakes. Indications that it has not yet done that include the Department’s response to the Parliamentary and Health Service Ombudsman’s recommendations on 1950s women, repeated problems with communications about changes to the State Pension and the delay in progressing the action plan.
