In the my circumstances it is my statutory duty to report to you _ Mr. Browning was discharged from Somewhere House without his family being informed of arrangements for his accommodation Heowas also discharged on a Friday, as a result of which there Was little time to accommodation was provided, and no proper accommodation was found ensure for him unclear as to whether Somewhere House routinely ensured The evidence was even where a client consented to this_ family involvement before discharge Teheievidencerestablished the importance of discharge and accommodation arrangements in vulnerable clients such as Mr: Browning

On 31st August 2012 an_investigation commenced into the death of Alan Stapfield BROWNING , Aged 43_ The investigation concluded at the end of the inquest on Tuesday 26th November 2013 The cause of death was 1a) Multiple Injuries and the conclusion of the inquest was Suicide
Circumstances of the Death
Mr: Browning was discharged from Somewhere House on 10" august 2012after treatment for alcohol and abuse: CCTV showed him jumping from Clifton Suspension Bridge on 16"h August 2012
