The Human Tissue Authority has expanded the scope of its adverse events and reportable incidents systems in the Post-Mortem sector to include the Anatomy sector. The HTA has issued formal guidance to ensure that adverse incidents in the anatomy sector are routinely reported. The reporting system went live on 1 December.
