Recommendation 51: Instruction on completing form XF001

From Report of the Zahid Mubarek inquiry (Volumes I and II), published 29 June 2006.

Recommendation

Staff who are tasked with completing form XF001 should be instructed on how to complete it by a senior officer. In particular, they should be reminded that they are only assessing the risk prisoners pose to other inmates. They should not automatically assess prisoners as a high risk simply because they claim, for example, to be prone to lose their temper, but should ignore such claims if they believe them to be untrue, and they should guard against being over-defensive.
