Recommendation
The Panel recommends the use of command and control systems to record operational decisions and actions at the scene, allowing the decision-maker and any other officer to check that each task has been performed and recorded.
Context
The recommendations chapter was prepared in 2011. The report states that no further evidence for this chapter was received after 2011 and that some recommendations may have been overtaken by the time of publication in September 2026. Implementation has not been assessed by this import.
