Independent investigation diagnosing the state of the NHS in England. Found the NHS was in critical condition with falling performance across virtually all metrics, chronic underinvestment in primary and community care, and workforce shortages.

Independent review of gender identity services for children and young people in England. Found the service provided by the Tavistock was not safe or viable, with insufficient evidence for the use of puberty blockers in under-18s.

Public body review examining the IOPC's efficacy, efficiency, governance and accountability. Made 93 recommendations, 73 addressed to the IOPC itself and the remainder to the Home Office and other bodies.

Independent review examining the quality and use of police data on race and ethnicity in the exercise of police powers including stop and search, use of force, and arrests.

Independent review examining how employers recruit, retain and develop autistic people, identifying barriers to employment and making 19 recommendations. Only around 30% of autistic working-age people are in employment.

Independent review examining whether civil service governance and accountability arrangements support effective government delivery. Made 57 recommendations across accountability structures, performance management, ministerial direction, and transparency.

Independent review examining how to improve the UK's offer to foreign direct investors. Made 6 recommendations to strengthen coordination across government and make the UK a more attractive investment destination.

Independent review examining the effectiveness of the disclosure and barring regime, covering safeguarding checks for aid workers, door supervisors, local councillors, and the international portability of DBS checks.

Independent review into the culture and standards of the Metropolitan Police Service. Found institutional racism, misogyny, and homophobia within the Met, with a culture of cover-up and denial.

Independent review examining the mix of UK organisations performing research, development and innovation. Made 29 recommendations to strengthen the RDI landscape and improve the UK's scientific and economic performance.

Independent review of the Prevent counter-terrorism programme. Found Prevent had been too focused on right-wing extremism at the expense of Islamist extremism, and had become too cautious about engagement with Islamist groups.

Independent review of the UK government's approach to delivering its 2050 net zero target, examining whether policies are pro-business and pro-growth. Made 129 recommendations across ten missions including a '25 by 2025' framework of priority actions.

Independent review of the Civil Aviation Authority examining its effectiveness, governance and regulatory approach, making recommendations to improve the regulator's performance, transparency and capability.

Independent investigation into serious maternity and neonatal service failings at East Kent Hospitals University NHS Foundation Trust between 2009 and 2020. Examined care provided to over 200 families and found a pattern of poor care, lack of compassion, and failure to respond to challenge.

Independent review examining the bureaucratic burden on UK researchers. Made 28 recommendations to reduce unnecessary administrative load on academics and streamline research governance.

Independent review commissioned by the Attorney General following the Court of Appeal's judgment in R v Akle & Anor, examining what went wrong in the SFO's handling of the Unaoil bribery investigation, including record-keeping, compliance with policies, and case assurance.
